I certify that I am not in default on a Federal Title IV or State of South Carolina educational loan, nor owe a refund on a Federal Title IV or State of South Carolina student financial aid program; that I have not been adjudicated delinquent; that I have not been convicted or pled guilty or nolo contender to any felonies or any alcohol or drug-related offense under the laws of this or any other state or under federal laws of the United States in order to be eligible for LIFE, Hope, or Palmetto Fellows scholarship; that I have not been convicted of any felonies and have not been convicted of an alcohol or drug-related misdemeanor offense during the preceding calendar year defined as 12 months from the date of the start of the school for period of this award; that I am a South Carolina resident; that I will be a full-time degree-seeking undergraduate student; (for incoming freshman) that I have a cumulative "B" average in high school under the Uniform Grading Scale. I have submitted previous and current official transcript(s) for all institutions attended; I understand that all previous and current grades received at institutions will be used in the calculation of my LIFE GPA. I hereby give permission for a background check to be conducted to verify the above. I understand additional information may be requested after the background check has been conducted.

Any false information provided by the student or any attempt to expend any scholarship funds for any unlawful purpose or any purpose other than in payment or reimbursement for the cost of attendance at the institution authorized to award the scholarship and/or grant will be cause for immediate cancellation. Any student who has obtained a scholarship through means of a willfully false statement or failure to reveal any material fact, condition, or circumstances affecting eligibility will be subject to applicable civil or criminal penalties, including the retroactive loss of the scholarship and/or grant.

As a South Carolina Tuition Grant recipient, I affirm that I have never been convicted, pled guilty, or pled no contest to any felonies nor have I ever been convicted, pled guilty, or pled no contest to an alcohol or drug related misdemeanor offense under the law of this or any other state or under the laws of the United States during the preceding calendar year defined as 12 months from the date of the start of school for the period of this award. If this status should change after I have signed the affidavit or by the start of the school, I agree to notify the Financial Aid Office immediately.

I certify that I will not engage in the unlawful manufacture, dispensation, possession, or use of a controlled substance during the period covered by my aid.
